At Tiara Jones Counseling & Case Management Services, we are dedicated to delivering professional counseling and case management services to adults and families. Our mission is to provide the tools and resources needed to help resolve personal problems and meet manageable goals. We believe that everyone needs help from everyone and that courage is not having the strength to go on; it is going on when you don't have the strength.

Our vision is to help adults and their families feel empowered to manage their stressors through supportive counseling and linkage to community resources. We aim to provide a safe and trusting environment so that we can collaborate to set and achieve manageable goals. We believe that helping people brings great joy, but helping people to help themselves is even more gratifying.

Our Services

We offer a variety of services to help our clients overcome their challenges and achieve their goals. These include:

Case Management

Case management is a method of providing services whereby a professional social worker collaboratively assesses the needs of the client and the client's family. When appropriate, the social worker arranges, coordinates, monitors, evaluates, and advocates for a package of multiple services to meet the specific client's complex needs. As a Licensed Clinical Social Worker, we address both the individual client's biopsychosocial status as well as the state of the social system and provide intervention at both the client (patient and family) and system levels.

Counseling

We provide one-on-one counseling sessions to help our clients process their feelings and address their stressors. Our goal is to build and maintain a therapeutic relationship with our clients and provide them with the support they need to manage their challenges.

Senior Case Management

We specialize in working with the adult and geriatric population. We have specialized training and experience in conducting assessments, crisis-intervention, family therapy, and case management services for seniors. Our goal is to provide a safe and trusting environment for our senior clients and help them navigate their unique challenges.

Resources

We also provide our clients with access to community resources and support. We believe that there are many resources available to help people, but many people do not know how to access them. We are here to connect our clients to the resources and additional support they need.
