Meet John DeBisceglie, PMHNP

John DeBisceglie is an Advanced Practice Nurse Practitioner (APRN) specialized as a Psychiatric Mental Health Nurse Practitioner (PMHNP). With over a decade of experience in outpatient settings, John has treated individuals across many phases of the life span. John prides himself in getting to know the person that is entering treatment, taking the time to listen carefully to the person's strengths, aspirations, and goals. John considers treatment a partnership and honors that he is invited into your world to help you achieve your treatment and life goals.

Preparing for Your First Appointment

To ensure that your first appointment is productive and efficient, we ask that you please read and complete the following forms prior to your visit:

• Treatment Agreement form
• Treatment Goals form
• Confidentiality acknowledgment form

We will review these forms together during your first appointment to clarify any questions or concerns you may have.
