Saint Francis Rehabilitation and Nursing Center is a 137 bed Skilled Nursing Facility located in the Grafton Hill area of Worcester. Our campus consists of 63 long-term care beds, including a 16 bed men's only unit. We provide a full range of healthcare services, treatments and therapies to meet our resident's diverse medical needs. Our staff of doctors, nurses, dietitians and social workers collaborate with the patient and his or her family to develop an individualized care plan to maintain and/or improve the quality of life. We offer a variety of programs, including Short-Term Rehabilitation, Long Term Care (LTC), Alzheimer's and Dementia Care, Pulmonary Care, Hospice and Palliative Care, Veterans Administration Contract, and Adult Day Care. Our goal is to provide a pleasant environment through a wide range of amenities and activities, catering to the physical, emotional and social needs of each resident.
