At Rosewood Rehabilitation and Care Center, we understand the importance of compassion and skill when combined into one. This is why we have dedicated ourselves to making the decision of choosing the right skilled nursing facility as simple as possible.

Located in Converse, Texas, a suburb of San Antonio, our center is close to several major acute care hospitals and residential neighborhoods. As a one-hundred twenty-five bed skilled nursing facility, we provide twenty-four hour residential health services for individuals who are no longer in an acute phase of illness but continue to require frequent nursing observation, care, and rehabilitative services.

Over the past 25 years, we have built and maintained a first-rate reputation as a vital part of the Converse community. We are proud to be owned by a Texas-based company, Senior Care Management, whose mission is to serve seniors across Texas in regional communities such as Converse. Our purpose is to help chronically ill or disabled residents reach and maintain their greatest potential and to continue a quality living experience adapted to their individual health status and personal preferences.

Our staff is dedicated to not only promote physical health but also to enrich the lives of our residents by encouraging functional ability in all ways; physically, emotionally, mentally, and socially. At Rosewood, our residents come first. We create a personalized plan of care that reflects each resident's specific needs. We get to know our residents and develop individual service plans that reflect their preferences, likes, and dislikes. Rosewood Rehabilitation and Care Center is here to assist you with your short-term, long-term, or respite needs, providing excellent and professionally trained staff who are here for one purpose: to serve you.

Our Rehab-to-Home Program is designed for individuals who need to continue their recovery after being discharged from the hospital. Whether you were in the hospital for a knee replacement, hip replacement, congestive heart failure, stroke, or one of many other reasons, we are here to help. We have great results, great therapists, wonderful nursing staff, and many years of experience helping people in your same situation get back on your feet and back into your routine at home. If you are over the age of 65, you most likely have Medicare Part A (hospitalization insurance). After specific criteria have been met, Medicare Part A will cover 100% of the cost of room and board, meals, and medications for the first 20 days of your stay while at Rosewood. While many will be able to reach their goals before day 20, some will need additional care. Medicare covers an additional 80 days at 80% and your secondary insurance (AARP, AETNA, or many others) will pick up the majority, if not all, of the additional 20% (2019 amount is $170.50 per day if paying privately).

We also offer long-term care for individuals who need ongoing support and care. There comes a time in one's life that decisions need to be made for the long-term safety and benefit of someone we love.
