• Nicole Haskins, Client Champion + Head of Hospital Contracts: Nicole has a Master's of Science in Health Promotion and has worked in various healthcare settings for over 10 years. She is passionate about ensuring that the aging population has access to the care they deserve as they transition from home to their next level of care.
• Chris Leaman, Transitions Manager: Chris is a licensed social worker with years of experience working in an acute care setting. He is dedicated to guiding patients and families through the transitions process with empathy and individualized care.
• Ashley Nokes, Process Improvement Manager: Ashley is a licensed social worker with 10 years of experience working with older adults in long term care settings. She is passionate about assisting medically complex older adults and their families as they navigate transitions to different levels of care.
• Debbie Falter, Transitions Specialist: Debbie has worked in healthcare with the senior population for the last 18 years. She brings a personal touch to her work, having cared for her own family members who suffered from Alzheimer's disease.
• Erin Oczkowski, Transitions Specialist: Erin is a Licensed Clinical Social Worker with 18 years of experience working with older adults and their families. She has a background in nursing homes, assisted livings, hospitals, and elder care management.
• Cristine Castle, Transitions Specialist: Cristine is a licensed social worker with 15 years of hospice and palliative care experience. She is dedicated to easing the transition to assisted living care during an emotionally challenging time.
