1. Download and Complete Notice of Claim: We've developed a simple physician's notice of claim form that you can download and complete. Please be sure to fill out the form carefully and attach any supporting documents.

2. Contact Our Claims Department: If you have any questions or concerns about the claims notification process, please feel free to contact our claims department. We're always available to review the claims notification procedures and provide guidance.

3. Mail Form and Required Documents: To ensure that we receive your form and supporting documents, we recommend mailing them via USPS, FedEx, or UPS. This will provide you with a receipt and tracking opportunities.

New York State Excess Liability Program

The New York State Excess Liability Program, also known as Section 18, is an important program for healthcare professionals in New York State. As an insurance agency, we're committed to staying up-to-date with changes in legislation and providing our clients with the latest information on this program. Changes in Participation

As of April 2022, the state's budget appropriated $102.1 million to the excess medical malpractice pool for the 2022-2023 policy year. The Section 18 legislation for the 2022-2023 policy year is similar to the prior year, and those who were in the program last year (even for less than a full year) will qualify for the upcoming policy year. To qualify, you'll need to file through one hospital and one carrier. Your current excess carrier will mail an excess renewal application form for you to complete and return to the carrier.
