Who We Are

Continuum of Care, Inc. is a non-profit organization dedicated to empowering individuals with mental illness, developmental disabilities, and substance abuse issues to rebuild their lives and thrive in the community. Our journey began in 1966 with the establishment of The New Haven Halfway House, our first residential program for individuals with mental illness. Since then, we have evolved into one of the largest community housing providers in Connecticut, serving over 2,000 individuals annually across various locations.

Our Philosophy

At Continuum of Care, Inc., we believe that every individual deserves the right to live a meaningful life in the community, free from the constraints of institutions, hospitals, or jail. Our philosophy is rooted in the idea that with the right support and services, individuals can break the cycle of homelessness and institutionalization. We provide comprehensive residential case management, crisis services, and housing support services to adults challenged by psychiatric disabilities, developmental disabilities, homelessness, and substance abuse treatment needs.

Mental Health Services

• Acute Services: Our acute services provide immediate support and emotional care for adults experiencing mental health crises.
• Crisis Stabilization & Respite Services: We offer short-term stabilization and respite services to help individuals manage their mental health.
• COMPASS Mobile Crisis Services: Our mobile crisis services provide immediate support and crisis intervention in the community.
• Veterans Recovery Support Program: We offer specialized support services for veterans struggling with mental health issues.

Housing Services

• Community Housing: We provide a range of housing options, including group homes, supported living homes, and transitional housing.
• Long-Term Services: Our long-term services offer ongoing support and services to individuals with chronic mental illness.
• Transitional Services: Our transitional services help individuals transition to independent living and community integration.

Developmental Disability Services

• Developmental Disability Programs: We offer a range of programs and services designed to support individuals with developmental disabilities, including autism, traumatic brain injury, and seizure disorders.
• In-Home Support for Autism & Developmental Disabilities: Our in-home support services provide one-on-one support and supervision to individuals with developmental disabilities.
• Continuous Residential Supports: Our continuous residential supports provide ongoing support and services to individuals living in community-based settings.
