Operated since 1983, Colonial Poplin Skilled Nursing & Poplin Way Assisted Living Communities are dedicated to providing exceptional care to our residents. Our privately owned and operated Medicare and Medicaid certified nursing and rehabilitation center is among the best senior living communities in the state of New Hampshire.

We strive to meet the changing needs of our senior population by offering multiple levels of care under one roof, including assisted living, skilled nursing, rehabilitation, and long term care services.

Skilled Nursing & Rehabilitation Services

Our skilled nursing and rehabilitation services are tailored to meet the diverse cognitive and physical abilities of our residents. Our 50-bed Medicare and Medicaid Certified skilled nursing/rehabilitation home offers individualized services to ensure that our residents receive the best possible care.

We provide short term skilled nursing and rehabilitation services at the Colonial Poplin nursing facility, where patients receive daily treatment to achieve recovery and strength to avoid re-hospitalization after they've been discharged home. Our long term skilled nursing and rehabilitation services ensure that our residents live in a socially active senior living community that feels like home, while also receiving daily, individualized care from a friendly and caring staff.

Our skilled nursing services include prime HMO affiliations, physical, occupational and speech therapy up to seven days a week, respiratory therapy, advanced wound care management, cardiopulmonary program, treatment of joint replacements, sub-acute services including IV therapy, PICC, midline, peripheral, central and portacath, pain management, BI-PAP and C-PAP oversight, geriatric mental health services, diverse life enrichment program in-house and off-site with our own bus, respite care, staff dietician, and full service cafe.

Assisted Living at Poplin Way

Poplin Way is a socially active Assisted Living Community that offers around the clock nursing assistance in a charming, country environment. Our residents live in their own apartment, with the added benefit of having convenient access to skilled nursing care in the same location, when needed.
