- What is the referral process?
+ A social service director will fax to SCG, LLC a signed primary care physician order, "resident face sheet, " and our proprietary resident symptom checklist.
- How are the primary care physician and nursing home staff "kept in the loop?"
+ Customarily, an interdisciplinary meeting between the provider, the social service director, and staff is held every quarter. The primary care physician can attend these meetings or receive an update from the Social Service Director at the conclusion of the meeting.
+ Additionally, our providers are in regular contact with social service directors so emergent issues/challenges are communicated weekly.
