Our mission is to provide exceptional mental health and primary care services to individuals living in nursing homes, assisted living facilities, and the community. We are committed to delivering timely and effective care that addresses the unique needs of each patient.

Our Services

Our services include individual psychotherapy, psychiatric medication management, and facility-based primary care. Our team of licensed professionals, including PhDs, PsyDs, LCSWs, and LISWs, provide mental health evaluations, staff consultations, and 1:1 therapeutic counseling. Our psychiatric team, comprised of physicians, physician assistants, and nurse practitioners, offers psychiatric evaluations and medication management services.

We also provide routine and acute visits to residents of assisted living and nursing facilities. Our providers treat residents onsite at the facility and become part of the treatment team. Regular visits to each facility eliminate the need for patients to be transported off-site, making it more convenient for patients to receive the care they need.
