Our Mission

Our mission is to provide comprehensive, accessible, timely, and affordable primary health care, preventive education, and social service programs to the Greater Englewood and surrounding communities. We believe that everyone deserves access to quality healthcare and social services, and we strive to make our services available to everyone, regardless of their ability to pay.

Our Services

We offer a wide range of services, including medical and dental care, COVID-19 screening and testing, and transportation to and from our facilities. Our medical services include primary care, preventive care, and chronic disease management, as well as specialty services such as podiatry and ophthalmology. Our dental services include routine cleanings, fillings, extractions, and oral health education. We also offer a sliding fee discount scale payment plan based on total family income and size to ensure that our services are affordable to everyone.

We accept Medicaid and Medicare, as well as many major health plans, and we offer a payment plan for those who are uninsured or underinsured. Our Locations

We have two convenient locations: our main office at 6821 S. Halsted Street in Chicago, IL, and our satellite office at 2338 New Street in Blue Island, IL. Both offices are equipped with state-of-the-art technology and staffed by caring and dedicated healthcare professionals.
