Our Mental Health Outreach Clinic provides in-home therapy services delivered by licensed professionals including psychologists and clinical social workers. We serve adults with serious mental illness across the Twin Cities metro area, offering flexible scheduling with appointments available days, evenings, and weekends to accommodate diverse needs.
Health & Wellness Initiative
This initiative addresses critical health disparities faced by individuals with mental illness, focusing on reducing 25-year mortality gaps through tobacco cessation support and improved dental care access. Since 2014, we've integrated physical health into our mental health services, training staff to address co-occurring health needs and promote overall wellness.
Substance Use Disorder Treatment
Our co-occurring treatment program for substance use disorder, established in 2018, addresses both mental health and addiction challenges simultaneously. This integrated approach recognizes the complex relationship between mental illness and substance use, providing comprehensive care tailored to individual recovery goals.
Case Management
We offer holistic case management services that connect individuals with necessary community resources. Our approach emphasizes individualized planning and ongoing support to help clients navigate healthcare systems, access housing, and build sustainable recovery pathways through coordinated care.
Anti-Racism Programs
Our anti-racism initiatives focus on dismantling systemic barriers in mental health care through staff training, policy reform, and community engagement. We actively work to reduce health disparities for BIPOC communities by addressing social determinants of health and promoting inclusive practices across all our services.
