Our mission is to provide coordinated, comprehensive services to meet the care needs of the individual resident and patient. Our interdisciplinary team members work together to develop an individualized plan of care for each resident and patient. We strive to meet or exceed our customers' expectations each and every day, promoting our core values of Communication, Compassion, Dignity, Empathy, Integrity, Pride, Respect, and Responsiveness.
Our vision is to be responsive to the ever-changing needs of the communities we serve. We facilitate resident, patient, and family involvement and integrate our services into the community to become the provider of choice for long-term care. We foster an environment that encourages learning and embraces diversity to support the professional growth and development of our staff members.
Our Services
We offer a range of services, including post-acute services, rehabilitative services, skilled nursing, short-term and long-term care. Our team of professionals includes Physical, Occupational, and Speech Therapists; Registered and Licensed Practical Nurses; and Certified Nursing Assistants. We also provide Social Services, Activities, Nutritional Services, Housekeeping, and Laundry Services.
Our Post-Acute Services
Our post-acute services assist those transitioning from hospital to home. Our team of professionals works with the physician to provide the level of care and services that will help each individual meet their goals for a successful discharge home.
Long-Term Care
For those residents who can no longer be on their own and need long-term care, we offer general nursing and restorative services in a warm and home-like environment.
