At Louisiana Health and Rehab (LHR), we are dedicated to providing a comprehensive range of services to individuals across the lifespan, from birth to the elderly. Our mission is to provide quality, integrative services to those struggling with substance use disorders, mental health issues, homelessness, and HIV/AIDS.
Our History
Founded by Soundra Temple-Johnson, LHR is a non-profit organization with a rich history dating back to 2005. With over 20 years of experience, we have established ourselves as a leading provider of behavioral health services in Louisiana. Our organization is comprised of two distinct programs: LA Health and Rehab Center, Inc. (LHRC) and LA Health and Rehab Options, Inc. (LHRO).
Our Services
At LHR, we offer a range of services designed to support individuals with diverse needs. Our services include:
- LHRC Reality House: A residential treatment program for pregnant women and their dependent children aged 0-12.
- LHRC Life Care Center: An outpatient treatment center providing intensive and outpatient services for substance abuse treatment.
- LHRC Fetal Alcohol Spectrum Disorder (FASD) Screenings and Modifications to Treatment (SMT): A pioneering program providing evidence-based treatment for individuals with FASD.
- LHRO New Opportunities Waiver (NOW), Community Choice Waiver (CCW), Long Term Personal Care Services (LT-PCS), and Children's Choice (CC): A combination of home and community-based services for the developmentally, intellectually, and physically impaired of all ages.

Our Vision

Our Mission
Our mission is to develop and successfully implement comprehensive quality residential and home and community-based services to assist those with substance abuse and mental illness in becoming productive, self-sufficient, and contributing members of society. We aim to maximize independence and quality of life for the elderly and the emotionally, intellectually, and physically disabled.
Overall Experience
The reviews describe the company as either extremely negative or extremely positive, with no middle ground. Two reviewers had a terrible experience, while one reviewer had an exceptional experience.
