At Aza Health, we have a long history of providing comprehensive healthcare services to individuals and families across northern Florida. Founded in 1977 as Rural Health Care, Inc., we have evolved into a private non-profit community and migrant health center that operates in areas designated as Health Professional Shortage Areas and or Medically Underserved Areas or Populations.

As a federally qualified health center (FQHC), Aza Health is committed to delivering high-quality healthcare to those who need it most. Our dedicated team of healthcare providers, including board-certified and board-eligible physicians in Family Practice and Internal Medicine, work tirelessly to ensure that our patients receive the best possible care. We offer a wide range of comprehensive services to keep you healthy throughout your life, including medical and dental care for every stage of life, mental health counseling for individuals, families, and groups, and convenient scheduling for same-day appointments, Telehealth visits, or scheduling your in-person appointment in advance.
