With the Patient Portal, patients can view their health summary from the Long Valley Health Center, request prescription refills, review laboratory test results, and access other important health information. The Patient Portal is a free service offered to all patients of the Long Valley Health Center.

The Long Valley Health Center is a not-for-profit organization that has been providing quality primary care to the people of Northern Mendocino County since 1978. Our mission is to improve the quality of life for all people of Northern Mendocino County by providing, in a respectful and compassionate manner, the most comprehensive and highest quality, integrated health care services possible. We are committed to increasing health and wellness in the Laytonville and surrounding communities by providing quality healthcare and increasing access to health services.

Our clinic is located at 50 Branscomb Rd. Laytonville, CA 95454 and our dental clinic is located at 51 Branscomb Rd. Laytonville, CA 95454. We have a team of experienced healthcare providers who are dedicated to providing the best possible care to our patients. Our team includes physicians, nurse practitioners, physician assistants, behavioral health specialists, dentists, and other healthcare professionals. We offer a wide range of services, including behavioral health, case management, chiropractic, chronic care, coronavirus (COVID-19) vaccine, coronavirus (COVID-19) testing, dental services, medically assisted therapy, pediatrics, primary care, remote patient monitoring, support groups, telemedicine, and women's health.

The Long Valley Health Center serves the people of Northern Mendocino County. We are committed to providing quality healthcare to all members of our community, regardless of their ability to pay. We accept a wide range of insurance plans, including Medi-Cal, Medicare, and private insurance. We also offer a sliding fee scale for patients who are uninsured or underinsured.
