Lakeshore Community Health Care

Lakeshore Community Health Care is a patient-centered medical home dedicated to providing comprehensive, integrated health care to everyone, regardless of insurance status or income level. We believe that everyone has a right to quality health care, and we strive to eliminate health disparities among the underserved. Our skilled providers focus on each patient's unique needs, and we offer a range of services including primary and preventive medical, behavioral (mental) health, dental care, and pharmacy services. We also provide support services and community resources to ensure that our patients receive the best possible care.

Our Services

We offer a variety of services to cater to the diverse needs of our patients. Our dental services include routine check-ups, cleanings, fillings, and extractions. Our medical services range from routine check-ups to chronic disease management. We also provide behavioral health services, including counseling and therapy, as well as chiropractic care. Additionally, we have a pharmacy on site and offer support services such as transportation assistance and interpreter services to ensure that our patients can access the care they need.

Recent Initiatives

We are currently implementing the evidence-based Columbia Suicide Severity Rating Scale to address the alarming rise in suicide rates. We are also transitioning the patient records of Dr. James E. Albrecht Free Clinic to our organization. These initiatives demonstrate our commitment to addressing the most pressing health issues in our community.
