At LiveActive, we are dedicated to changing the experience and delivery of primary care. Our mission is to increase accessibility, affordability, and personalization in healthcare. We envision becoming the leading personalized health care service for individuals in the community and for employers. We believe that by providing primary care at home, in our office, and virtually, we can improve access to healthcare and prevent readmissions to hospitals and skilled nursing facilities.

After over 10 years of practicing medicine, we have learned that primary care is key to preventing unnecessary hospital visits and improving overall health outcomes. We leverage our knowledge of community resources to bring you the best care our community can offer. We are passionate about serving our patients and helping them navigate the complex healthcare system.

Our Services

We offer primary care services in various settings including our private outpatient office in Overland Park, KS, Assisted Living Facilities, Independent Living Facilities, Private Homes (case by case basis for members only & within a 5-mile radius from our outpatient office), and Telemedicine via videoconferencing services. We also provide Inpatient Hospital service for those who need it.

How to Get Our Services

You can request our outpatient primary care services by clicking here: "JOIN NOW". Alternatively, you can be referred to us by your PCP, specialist, or hospital care coordinators, or by a discharging skilled nursing facility or rehab facility for medical necessity. Family members and caretakers are also welcome to request our service for their loved ones if there is a medical necessity.

Our Availability

We have limited availability for accepting new patients to assure we provide the best care. Our clinic hours are Monday to Friday from 9am to 12pm, with daily closure from 12pm to 1pm for lunch. We are closed on Saturday and Sunday. Please note our holiday closures for New Year's, Memorial Day, Independence Day, Labor Day, Thanksgiving, and Christmas.
