Do I Need a Doctor's Referral?

Most insurances require a doctor's referral in order to attend physical therapy. We recommend checking with your insurance provider to determine their specific requirements.

Types of Health Insurance Accepted

We accept all major insurance providers, including BCBS, Moda, Pacific Source, Providence, Atrio, Cigna, and Healthnet. Please note that as of December 2023, we will no longer be accepting new patients with Wellcare from Healthnet. If you have any questions or concerns, please don't hesitate to contact us.

New Patient Procedure

When you arrive for your initial appointment, please bring your ID and insurance information. We will also have some paperwork for you to fill out, so we recommend arriving about 15 minutes early to complete the necessary forms. Our front office staff will be happy to assist you with any questions or concerns you may have.
