At Sunflower Communities, we are dedicated to providing exceptional assisted living and memory care services throughout Minnesota. Our organization operates two distinguished locations: Elmore Assisted Living in Elmore, MN and Nagel Assisted Living in Waconia, MN. Our Elmore community features six neighborhoods, including dedicated memory care floors, situated on an expansive property with outdoor courtyards and gardens. The Nagel community resides in the renovated former Nagel Hospital, blending historic charm with modern amenities and newly constructed memory care areas.

Onsite health services include primary care, pharmacy, behavioral health, occupational, speech, and physical therapy, supported by care coordination. Residents benefit from engaging activities such as daily exercise, music therapy, pet therapy, arts and crafts, social parties, and community outings. Our innovative memory care approach follows the Seven Petal Plan and the SEEDS OF SUCCESS program to address dementia with personalized, compassionate care.
