Our Communities

We have two beautiful communities, Elmore Assisted Living and Nagel Assisted Living, both located in Minnesota. Elmore Assisted Living is a pet-friendly community featuring 6 neighborhoods, including memory care on 3 floors, situated on an expansive property with an outdoor courtyard that includes gardens and a pet area. Nagel Assisted Living, located in the heart of downtown Waconia, is housed in the former Nagel Hospital and has been freshly renovated to evoke nostalgia of years gone by, coupled with modern amenities in our newly constructed memory care and community living areas.

Our Services

Our communities offer a range of services and amenities designed to meet the unique needs of our residents. These include engaging activities, delicious dining options, onsite services such as healthcare, therapy, and salon services, and memory care programs tailored to meet the specific needs of our residents with Alzheimer's and dementia. We also proudly accept residents on CADI & BI Waiver, ensuring that our services are accessible to those who need them most.
