Our Mission and History

Riverbend Nursing and Rehab Center has been serving Plaquemines Parish since 1999, and we are committed to providing the highest quality of medical care and rehabilitative services. Our mission is to provide a caring, comfortable, and clean environment where our patients receive the specialized rehabilitation and skilled nursing services they need. Our experienced and highly trained rehabilitation and nursing staff are dedicated to helping our patients achieve their short-term transitional goals and long-term quality of life needs.

Our Services

At Riverbend Nursing and Rehab Center, we offer a wide range of services designed to help our patients recover and regain their independence. Our services include:

• Rehabilitation to Home: Provided up to 6 days a week by licensed professionals, our rehabilitation services include physical, occupational, and speech therapy.
• Transitional/Short-Term Care: Our skilled nursing and rehabilitation model provides transitional care and short-term stays while you recover and regain independence.
• Community Re-entry: We help our patients reintegrate into their communities with our family training and involvement program.
• Family Training and Involvement: We encourage family involvement and participation in our patient's care and recovery.
• Dedicated Rehabilitation Space: Our rehab gyms provide well-equipped space for patients to receive therapy with state-of-the-art equipment.
• Memory Lane: Our secure Alzheimer's/Dementia Unit, Memory Lane, is designed to provide a safe and peaceful environment for patients with Alzheimer's disease or other forms of dementia.
• 24-Hour Skilled Nursing Care (SNF): Our team of skilled nursing and rehabilitation professionals deliver physician-prescribed medical interventions for complex medical conditions.
• IV Therapy: We offer IV therapy for our patients who require additional medical support.
• Wound Care Management: Our skilled nursing staff manages wound care for our patients with complex wound care needs.
• Laboratory: We have an on-site laboratory for quick and convenient testing.
• Physician Directed Services: Our services are supervised by medical professionals who develop and implement individualized care plans for each patient.
• Social Services: We offer social services to support our patients' emotional and psychological well-being.
• In-House X-Rays: We have an on-site x-ray machine for quick and convenient imaging services.
• Secure Care Safety System: Our safe and secure environment is designed to ensure the well-being and safety of our patients.
• Transportation Services: We offer transportation services to and from our facility, making it easier for patients to access our care.
• Semi-private suites: Our comfortable and clean private suites provide a relaxed and calming environment for our patients.
• Free Wi-Fi Access: We offer free Wi-Fi access to our patients, allowing them to stay connected with loved ones and access important information.

Our Rehabilitation Model

Our rehabilitation model is based on a providing more therapy per day and more days per week to expedite your transition back home. Our highly skilled rehabilitation staff includes dedicated Physical, Occupational, and Speech therapists. Our rehabilitation model is rooted in proven clinical pathways, offering a range of customized treatment approaches, therapies, and services that few other programs can match. Our treatments are carefully planned and supervised by medical professionals in a rehabilitation environment guided by our compassionate team of nurses, therapists, and staff who will be with you every step of the way.
