Community Health & Wellness Center (CHWC) is dedicated to keeping communities healthy in northwest Connecticut. We provide compassionate, high-quality healthcare services in a patient-centered environment without discrimination. Through collaboration, advocacy, and responsible fiscal management, we aim to ensure the sustainability of our services for generations to come.

Services

CHWC offers a comprehensive range of healthcare services, including:

Primary Care: General check-ups, vaccinations, preventive screenings, and chronic disease management.

Women's Health: Comprehensive women's healthcare including contraception, prenatal care, and menopause management.

Nutrition Services: Individual and group counseling for weight management, healthy eating, and nutrition education.

Diabetes Prevention & Education: Programs and resources to prevent and manage diabetes.

Behavioral Health: Mental health screenings, counseling, and medication management.

Additional Services: Chiropractic care, podiatric care, dental care, pharmacy services, and mobile medical outreach through our BETTY unit.

Collaboration & Advocacy

CHWC actively collaborates with other organizations to expand access to care, advocate for healthcare policy, and foster community partnerships. We are proud to be a Federal Qualified Health Center (FQHC) receiving funding from the Health Resources and Services Administration (HRSA) and other government agencies.
