Our Story

Functional Assessment Rehab started in 1986 as Dell C. Felix, PT & Associates and in 2007 became Functional Assessment Rehab. We have consistently focused on providing a service to the community that emphasizes functional abilities and return-to-work options. Our team of experienced physical therapists and rehabilitation specialists are dedicated to helping individuals recover from injuries, improve their functional abilities, and return to their optimal level of independence and productivity.
