Our Services
Our services include Mental Health Targeted Case Management, which helps adults with serious and persistent mental illness (SPMI) and children with severe emotional disturbance (SED) gain access to needed medical, social, educational, vocational, financial, and other necessary services. We also provide Parenting Support Services, designed to assist parents in effectively working to improve their ability to parent and strengthen the parent-child relationship. Our case managers meet with individuals and families to assess needs, identify strengths, advocate for and connect to appropriate services, and monitor progress for continuity of care.

We provide services throughout Volusia, Flagler, Orange, Seminole, Duval, and Clay Counties. Our dedicated team of professionals is committed to delivering quality care and support to individuals and families in need.
