Our clinic was founded by David and Angela Carter, two dedicated professionals who are passionate about helping others achieve optimal health and well-being. Insurance and Patient Forms

We understand that navigating the complexities of insurance can be overwhelming. That's why we accept a variety of insurances, including Medicare, Humana, Aetna, United Healthcare, Anthem/BCBS, and Virginia Premier. Please bring your insurance card and PT prescription to your first appointment.

For new patients, we request that you fill out our patient registration forms prior to your visit. These forms can be found on our website or obtained in person at our clinic. Please also bring your insurance card and PT prescription from the referring physician.
