Clinical Liaisons

We understand that transitioning from hospital to home can be challenging, which is why we have Clinical Liaisons stationed at each of the major hospitals around the state. They ensure that our patients receive continuity of care, coordinate transitions from hospital to home, and provide disease state education and teaching to both patients and their families.

Who Pays for Home Health Care Services?

We accept payments from Medicare, Medicaid, and managed care/insurance plans. Medicare covers home healthcare services if our patients meet certain criteria, including their doctor verifying that they need medical care at home, requiring skilled nursing, physical therapy, occupational therapy, or speech-language therapy, and being homebound.
