Clinical Services

• Subacute Rehab: Our subacute rehab program is designed to help patients recover from surgery, illness, or injury. Our multidisciplinary team of healthcare professionals works together to develop a personalized treatment plan that addresses the patient's medical, physical, and emotional needs.

• Cardiopulmonary Care: Our cardiopulmonary care services are designed to help patients recover from heart failure, chronic obstructive pulmonary disease (COPD), and other cardiopulmonary conditions.

• Heart Failure Certification: Our program is certified in heart failure care, which means that our staff has received specialized training in caring for patients with heart failure.

• Chronic Kidney Disease Management: Our chronic kidney disease management program helps patients with end-stage renal disease (ESRD) or chronic kidney disease (CKD) manage their condition and maintain their quality of life.

• Long Term Care: Our long-term care program is designed to provide ongoing care and support for patients with complex medical conditions, including dementia, Alzheimer's disease, and other cognitive impairments.

• Respite Care: Our respite care program offers temporary relief for caregivers who need a break from their caregiving responsibilities.

Telemedicine Services: We offer telemedicine services to patients who require ongoing care and monitoring but cannot physically attend our facility.

Care Navigation: Our care navigation services help patients and their families navigate the healthcare system, ensuring seamless transitions between hospitals, rehabilitation centers, and long-term care facilities.
