Our facility is designed to provide a safe and sanitized environment, with Hepa filter-compliant exam rooms that ensure 12 times air changes per hour and no recirculation of air.

Our Team

Our team is led by Dr. Mohammed R. Azaz, a board-certified internal medicine physician with over 20 years of experience. Dr. Azaz is affiliated with St. Catherine of Siena Hospital in Smithtown and is on staff at St. James Nursing Home. He is dedicated to providing exceptional care to his patients, and his clinical area of interest is in taking care of and coordinating care of adult patients with a wide range of medical needs.
