Our History

LT Home Healthcare is an agency providing long-term home healthcare services. Our office is located in Conroe, TX, and we have been proudly serving Medicaid clients since December 2016. We are currently serving more than 600 clients throughout Texas.

LT Home Healthcare was established in 2016 when we felt the need to have a quality home healthcare agency that has the passion for our client's care. Even with 18+ years of combined hands-on experience in the home health care industry, we felt that we could not do it alone; thus, we have built a strong management team who has the same passion, values, and goals.

Our Services

We offer a range of services designed to meet the unique needs of our clients. Our services include:

• Family Care (FC): Our Family Care service provides assistance with daily living activities such as bathing, dressing, grooming, and toileting. We work closely with our clients and their families to develop a personalized care plan that meets their specific needs.
• Personal Care Service (StarKids): Our Personal Care Service is designed for children and young adults with special needs. Our team of caring professionals provides compassionate care that is tailored to the unique needs of each child.
• Primary Home Care (PHC): Our Primary Home Care service provides comprehensive care for clients who require assistance with daily living activities such as bathing, dressing, grooming, and toileting. We work closely with our clients and their families to develop a personalized care plan that meets their specific needs.
• Private Duty Nursing (PDN): Our Private Duty Nursing service provides skilled nursing care for clients who require assistance with medical needs such as medication administration, wound care, and catheter care.
• Respite Care Services: Our Respite Care services provide temporary relief for family caregivers who need a break from their caregiving responsibilities. We offer flexible respite care options that are tailored to the unique needs of each family.
